    Food Insecurity Experience: Building Empathy in Future Food and Nutrition Professionals

    OBJECTIVE To assess changes in empathy in students completing a food insecurity experience. DESIGN Mixed methods; quantitative data from survey in years 1 and 2; qualitative data extracted from students\' workbooks in years 2-5. This study was conducted over 10 weeks annually for 5 years. SETTING Northwest US land-grant university. PARTICIPANTS Students enrolled in a community nutrition course who chose to complete the food insecurity exercise. Total included 58 students in quantitative analysis in years 1 and 2 and 119 in qualitative analysis, years 2-5. INTERVENTION(S) The intervention was a food insecurity experience in which participants spent no more than $3/d on food for 5 days ($15 total) while striving for a nutritious diet and reflecting on their experience. MAIN OUTCOME MEASURES Empathy scores measured by Likert scales; participant responses and reflections recorded in workbook journals. ANALYSIS Comparison of means across time using paired t tests (P < .05); coding and sorting themes from workbook journals. RESULTS Quantitative findings indicated that both classroom content and experiential exercises were important for enhancing empathy about food insecurity. Empathy scores increased from time I to time II and from time I to time III. Qualitative reflections among participants included terms such as guilt, empathy, compassion, and raised consciousness about food insecurity. CONCLUSIONS AND IMPLICATIONS Experiential and transformational learning to develop empathy can take place in a 5-day food insecurity experience during a typical university-level community nutrition course. This intervention can be tested for applications in other contexts.

    Physical Activity and Inflammation Phenotype Conversion

    Inflammation is a protective response to infection or injury; however, persistent microtraumas at the tissue level may result in chronic low-grade inflammation that plays both direct and indirect roles in the development of many diseases and aging. The purpose of this review is to describe the underlying physiology of low-grade inflammation and highlight potential inflammation lowering effects of physical activity (PA). Unique contributions of this review are to introduce the concept of inflammation phenotype flexibility in contrast to the low-grade inflammation state and describe how PA influences inflammation phenotype by altering muscle, gut, adipose, and postprandial metabolism. Pro-inflammatory M1 macrophages and cytokines—such as tumor necrosis factor (TNF)-α, interleukin (IL)-1β, and IL-6—contribute to low-grade inflammation. Among the mechanisms that commonly contribute to low-grade inflammation are dysfunctional adipose tissue, a leaky gut, gut microbiota that promotes inflammation, and large postprandial glycemic and lipidemic responses. Physical activity may lower inflammation by decreasing M1 macrophages in visceral adipose tissue, decreasing adipose tissue volume, production of anti-inflammatory myokines, promotion of butyrate-producing members of the gut microbiota, improved gut barrier function, and lowering of postprandial glycemic and lipidemic responses. While exercise has many anti-inflammatory mechanisms, phenotype conversion is complex, multifaceted, and difficult to achieve. Our understanding of how PA influences inflammation must include acute exercise-induced anti-inflammatory effects, contribution to the inflammation state from multiple sources in the body, and phenotypic shifts underpinning low-grade inflammation.

    Genetic Diversity in Nutritious Leafy Green Vegetable-Chaya (Cnidoscolus aconitifolius)

    Chaya (Cnidoscolus aconitifolius ssp. aconitifolius Breckon) is a fast-growing, semi-perennial, and semi-woody Mesoamerican euphorbiaceous. It is used as a leafy green vegetable and prevailingly cropped in tropical savanna climate. However, cropping of chaya is possible in both dryer and more humid climates. Although the crop has its origin in the Maya region of Southeast Mexico, Guatemala, and Belize, chaya is popular throughout Mesoamerica. Due to its high nutritional value, cooked chaya leaves are an essential ingredient of the diet of Maya communities, especially in Southeast Mexico. Chaya is also used as an ornamental plant, for forage, and in traditional Maya medicine, where it is used to cure a wide range of diseases such as diabetes, kidney problems, arteriosclerosis, gallstones, and high cholesterol. Chaya can be called a semi-domesticated plant: Apart from wild chaya, there are four chaya varieties, whose grade of domestication varies from cropped almost wild phenotypes to entirely domesticated: ‘Chayamansa,’ ‘Redonda,’ ‘Estrella,’ and ‘Picuda.’

    Plant-Climate Interaction Effects: Changes in the Relative Distribution and Concentration of the Volatile Tea Leaf Metabolome in 2014–2016

    Climatic conditions affect the chemical composition of edible crops, which can impact flavor, nutrition and overall consumer preferences. To understand these effects, we sampled tea (Camellia sinensis (L.) Kuntze) grown in different environmental conditions. Using a target/nontarget data analysis approach, we detected 564 metabolites from tea grown at two elevations in spring and summer over 3 years in two major tea-producing areas of China. Principal component analysis and partial least squares-discriminant analysis show seasonal, elevational, and yearly differences in tea from Yunnan and Fujian provinces. Independent of location, higher concentrations of compounds with aromas characteristic of farmers’ perceptions of high-quality tea were found in spring and high elevation teas. Yunnan teas were distinct from Fujian teas, but the effects of elevation and season were different for the two locations. Elevation was the largest source of metabolite variation in Yunnan yet had no effect in Fujian. In contrast seasonal differences were strong in both locations. Importantly, the year-to-year variation in chemistry at both locations emphasizes the importance of doing multi-year studies, and further highlights the challenge farmers face when trying to produce teas with specific flavor/health (metabolite) profiles.

    Diversity and use of medicinal plants for soup making in traditional diets of the Hakka in West Fujian, China

    Background Wild edible and medicinal plants were an important component of traditional diets and continue to contribute to food security, nutrition, and health in many communities globally. For example, the preparation and consumption of soup made of medicinal plants for promoting health and preventing disease are a key component of the traditional diets of the Hakka socio-linguistic group of China’s West Fujian Province. As environmental and socio-economic factors drive the shift away from traditional diets, there is a need for ethnobotanical documentation of the diversity of wild edible and medicinal plants as well as associated knowledge and practices. Method Ethnobotanical surveys were conducted in Hakka communities in West Fujian Province between 2017 and 2018 to document plants used in medicinal soups as well as associated traditional ecological knowledge, practices, and conservation status. Surveys included semi-structural interviews, key informant interviews, participatory rural appraisal, and focus group discussions. Quantitative indices, including cultural food significance index (CFSI) and relative frequency of citation (RFC), were calculated to evaluate the importance of documented plants to Hakka communities. The species with the highest CFSI and RFC values were ranked by informants and further evaluated according to their individual properties and growth environment. Results A total of 42 medicinal plant species, belonging to 25 families and 41 genera, were documented for making soup by the Hakka. The Asteraceae botanical family was the most prevalent, and their root or the entire plant is used for soup making. Informants incorporate different ingredients in soups for their flavors as well as medicinal properties on the basis of the local ethnonutrition system. The most prevalent medicinal uses of the documented plants for making soups were used for clearing inner heat (58.1% of the species), treating inflammation (37.2%), and counteracting cold in the body (20.9%). Informants perceived that the medicinal properties of soup-making plants are influenced by the time of harvest, the local environment, and the climate. Conclusion Efforts are needed to preserve the ecological knowledge associated with traditional diets towards supporting both environmental and human well-being in rapidly developing communities experiencing the nutrition transition and biodiversity loss.

    Contribution of wild foods to diet, food security, and cultural values amidst climate change

    Wild foods are recognized to contribute to diet and food security through enhancing the availability of local, diverse, and nonmarket food sources. We investigated the contribution of wild foods to diet, food security, and cultural identity in a Native American[1] community in the context of climate change. Structured interviews were conducted with low-income residents of the Flathead Indian Reser­vation[2] in Northwestern Montana who participate in the federal Food Distribution Program on Indian Reservations, also known by participants as ‘Commodities.’ Responses to structured questions were analyzed for frequency, and open-ended responses were coded and analyzed to identify prevalent themes. Our analysis indicated that half of participants were food insecure. Approximately 28% of participants engaged in at least one wild food procurement activity, including hunting, fishing, and harvesting. On average, participants who engaged in one or more wild food procure­ment activities were more food secure than those who did not. Results highlight the multidimen­sional valuation of wild foods by participants including taste, freshness, nutritional quality, being a traditional community practice, and providing a sense of self-sufficiency. Climate change is per­ceived by participants to be adversely impacting wild food systems due to increased variability in seasonality and precipitation and increased inci­dences of wild fire. Findings point to the need for community-based strategies to strengthen wild food knowledge toward enhancing food sover­eignty in Native American communities, in the context of climate change. [1] The term ‘Native American’ was determined to be the preferred term for referencing the Native American community in this study, based on consultation from our community advisory board. [2] The term ‘Flathead Indian Reservation’ was determined to be the preferred term for referencing the location in which this study was held, based on consultation from our community advisory board.

    Urban Horticulture, from Local Initiatives to Global Success Stories

    Urban horticulture describes economically viable horticultural production activities conducted in a city or suburb. It is a growing segment of horticulture in the United States as well as in developing countries, where the enormous growth of megalopolis is not backed by a simultaneous increase of farmland or agricultural productivity. Today, urban horticulture includes food sovereignty in underprivileged neighborhoods, increased availability of vegetables and fruits in big cities, healthy and diverse diets, improved food safety, low transportation costs, efficient resource use, and the mitigation of environmental impacts of horticultural production such as the emission of greenhouse gases. The workshop “Urban horticulture: From local initiatives to global success stories,” held at the 2018 American Society for Horticultural Science (ASHS) conference in Washington, DC, featured present and historical success stories of urban horticulture from Asia, Africa, Latin America, and the United States.

    Sustainability Dimensions of a North American Lentil System in a Changing World

    Food production and consumption are among the largest drivers of global change. The adoption of lentil in production systems and in plant-based diets is a food system solution that can support the environmental, socio-economic, and human health dimensions of sustainability. The purpose of this study is to evaluate producer and consumer perceptions of the sustainability profile of the lentil system in Montana (USA), and the surrounding region that includes Idaho (USA), North Dakota (USA), Washington (USA), and Canada, in the context of global change. Surveys were conducted with lentil producers (n = 63; conventional n = 42, organic n = 15, and mixed management n = 6) and consumers (n = 138) in the rural state of Montana (USA). The most prevalent agronomic reason for including lentil in production systems reported by producers is to diversify crop rotation (92%). The most prevalent economic reasons for including lentil in rotation reported by producers is to capitalize on dryland production (95%) and to serve as a cash crop (87%). With respect to lentil consumption, the most prevalent health-related perceptions were that eating lentils helps to improve nutrition (88%), feel satiated or full (85%), and support a plant-based diet (81%). Consumers and non-consumers of lentils alike reported they would increase lentil consumption based on environmental (78%), economic (75%), and health and nutrition (72%) information contrasting lentils and animal-based protein sources. Overall, findings highlight how the lentil system supports multiple dimensions of sustainability based on the perspectives of study informants. Additionally, findings elucidate barriers and opportunities for promoting lentil in agricultural systems and diets. Impacts of market, policy, and climate change on lentil production, and lack of consumer knowledge on benefits of lentils to help meet food security through a sustainable diet, challenge sustainability dimensions of lentil in the food system.

    Dietary Beliefs and Management of Older American Indians With Type 2 Diabetes

    Objectives This qualitative study examined dietary-related beliefs and self-management among older American Indians with type 2 diabetes mellitus (T2DM). Design Semistructured in-person interviews were conducted and digitally recorded. Setting Southeastern American Indian tribal community. Participants A total of 28 noninstitutionalized older tribal members aged >60 years. Phenomenon of Interest Study participants’ beliefs and experiences with dietary practices and management related to T2DM. Analysis Transcribed qualitative interviews were coded using an inductive content analysis approach. Results Four themes regarding T2DM dietary beliefs and T2DM dietary management emerged from the analyses: diet changes, portion control, health care professional and family influence, and barriers to healthy eating. Study participants described how their beliefs, practices, and experiences in these 4 areas related to T2DM. Conclusions and Implications American Indian older adults face a variety of challenges to dietary management of T2DM. Future research efforts can focus on assessing how social support can be leveraged to facilitate healthy diets for American Indians with T2DM. Clinicians and diabetes educators and Native communities have an instrumental role in identifying culturally appropriate messages and programs to help persons effectively manage T2DM.

    The UnProcessed Pantry Project Framework to Address Nutrition in the Emergency Food System

    Low-income populations suffer a greater burden of chronic diseases and food insecurity, are more likely to consume ultraprocessed food, and are less likely to meet dietary recommendations than the general population.1,2 Ultraprocessed food is manipulated with artificial ingredients and can be high in sodium, caloric sweeteners, and saturated fats and, thus, has deleterious health effects.3 The NOVA framework, created by Monteiro and colleagues,3 is commonly applied to public health research to characterize food processing at different levels (unprocessed, minimally processed, processed culinary ingredients, processed, ultraprocessed).3 In the United States, ultraprocessed food typically is accessible, affordable, and convenient, whereas minimally processed food typically is more difficult to access, expensive, and less convenient.1,4 Over time, the availability of ultraprocessed food has increased across the global food supply and in the diets of individuals worldwide, especially among low-income popualations.
